Top 9 Symptoms of BVD According to 6,500 Responses

Binocular Vision Dysfunction (BVD) is a condition in which the eyes struggle to work together as a team. This misalignment can lead to a wide range of physical, visual, and cognitive symptoms that significantly affect quality of life. To better understand how BVD affects individuals, over 6,300 people completed the BVD Quality of Life Questionnaire.

The questionnaire includes 38 questions, divided evenly between distance-related symptoms (19 questions) and near-vision symptoms (19 questions). For each question, respondents rate the severity of their symptoms on a scale from 0 to 4:

  • 0 – Never

  • 1 – Rarely

  • 2 – Occasionally

  • 3 – Frequently

  • 4 – Always

This rating system allows clinicians and researchers to gauge how often and how severely BVD symptoms are experienced. Based on this data, we can identify the most common symptoms people report when living with this condition.

After analyzing responses from the BVD Quality of Life Questionnaire, clear patterns emerged. While BVD can present differently from person to person, some symptoms stood out as more frequently experienced across the board.

Here are the top symptoms based on how many people rated them as moderate (3) or severe (4):

1. Light Sensitivity (Glare)

  • Severe (4): 2,558

  • Moderate (3): 1,882

  • Total (3+4): 4,440

Light sensitivity ranks highest, with over two-thirds of respondents reporting discomfort with glare, especially from screens, headlights, or bright environments.

2. Fatigue or Sleepiness (Especially When Using Eyes)

  • Severe (4): 2,490

  • Moderate (3): 2,127

  • Total (3+4): 4,617

Visual fatigue, drowsiness, and frequent yawning while reading or working are strong indicators of BVD and are often mistaken for general tiredness or attention problems.

3. Neck or Shoulder Pain

  • Severe (4): 2,293

  • Moderate (3): 2,122

  • Total (3+4): 4,415

When the eyes are not aligned properly, people often unconsciously tilt their heads, which can lead to chronic neck and shoulder discomfort.

4. Feeling Spaced Out or Unfocused

  • Severe (4): 1,840

  • Moderate (3): 2,368

  • Total (3+4): 4,208

This is a common symptom. Many people describe a sensation of being disconnected, dazed, or struggling to stay focused.

5. Anxiety in Crowds or Stores

  • Severe (4): 2,550

  • Moderate (3): 1,764

  • Total (3+4): 4,314

Many individuals with BVD experience visual overload in busy environments, which can trigger anxiety or even panic attacks in crowds, shopping centers, or large open spaces.

6. Eye Discomfort (Burning, Itchy, or Watery Eyes)

  • Severe (4): 792

  • Moderate (3): 2,112

  • Total (3+4): 2,904

Irritated eyes are not always caused by allergies. Visual misalignment often leads to overuse of the eye muscles, which can cause these symptoms.

7. Headaches or Facial Pain

  • Severe (4): 940

  • Moderate (3): 2,180

  • Total (3+4): 3,120

Frequent tension headaches, migraines, or facial pain around the eyes or temples are often linked to constant visual strain.

8. Clumsiness or Poor Coordination

  • Severe (4): 1,464

  • Moderate (3): 1,825

  • Total (3+4): 3,289

Misjudging distances or bumping into objects is common and may be caused by poor depth perception or difficulty coordinating the eyes.

9. Reading Challenges (Skipping Lines, Skipping Words)

  • Skipping Lines – Severe (4): 1,310, Moderate (3): 2,344

  • Skipping Words – Severe (4): 805, Moderate (3): 1,764

These are strong signs of eye tracking and alignment issues, which are core features of BVD.

What This Data Tells Us

These findings show that BVD affects much more than just vision. From chronic fatigue and headaches to anxiety and memory challenges, the symptoms impact nearly every part of daily life.

Many people go years without a diagnosis. They are often treated for migraines, ADHD, or anxiety without ever being screened for BVD.
